- Transition Care Program
The UCLA Med-Peds Transition Care Program helps children with chronic conditions make a smooth transition to adult healthcare. Program experts provide consultation for adolescents and young adults (15 to 25 years old) requiring either specialized medical care for complex conditions and/or coordination of healthcare and community-based services.
- School Function Program
The UCLA School Function Program addresses the educational, learning, and mental health needs of children in school or day care.
The program helps to diagnose developmental and school challenges so the child may receive adequate treatment he or she needs in order to overcome negative barriers early in life.
- UCLA Center for Healthier Children, Families, and Communities
A multidisciplinary, community-focused research, policy, and training center. Focus is on Systems Innovation and Improvement, Research and Evaluation, Training and Model Programs, and Policy. This offers strategic, integrated techniques for transforming care and improving the long-term health of children and families.
